Workshop Selections

Payment: After August 1,we can only accept money orders made out to Phoenix Rising Productions. Pay for your 1st choice workshops. We'll send you a refund check if you overpay.

You can pay in full or in two installments. Send 1/2 of the total due now and remit the final 1/2 to reach us by May 15, 2005. No reminders will be sent. Should you fail to make the final payment by the due date, your classes will be dropped. Monies will not be returned; May 15th is past the cancellation refund date. PAYMENT PLAN NOT AVAILABLE AFTER MAY 1, 2005.

Confirmation: You may assume that your 1st choice workshops are confirmed if you don't hear from us. We will contact you if there is a problem with your registration. Keep a copy of this form for your records.

Schedule Changes will incur a $25 change fee. Change requests should be mailed along with the change fee. Sorry, no changes based on phone calls or email.

Cancellations prior to May 1st will incur a $50 processing fee. Cancellations after May 1st are non-refundable. The $10 registration fee is non-refundable. Sorry, no cancellations based on phone calls or email.

We recommend that you attach a separate sheet of paper with your alternate workshop selections.

Everyone attending 1 or more full day workshops will receive an ART Unraveled tote bag with logo, while quantities last. The quantity of gifts inside the bag will vary based on the number of workshops taken.
