To pay by Credit/Debit Card: you will receive an invoice once your registration has been received. We will hold your ticket(s) for 24 hours from the time of invoicing awaiting payment. Payment in full is required. You will receive a receipt and your ticket(s) will be mailed to the address provided on this form.
To pay via mailed check or money order (in U.S. dollars only): you will receive an invoice once your form has been received. We will hold your Gala tickets for 6 business days awaiting your mailed payment. PRP will issue a receipt. No checks accepted after July 1st. Money orders okay.
Returned checks will result in the cancellation of ticket(s). You must resubmit the form and include a $38 returned check fee with a new form and payment.
In the case of Acts of God, war, disaster or unforeseen circumstances, Phoenix Rising Productions reserves the right to reschedule this event.
